By submitting this form, you consent to London South Bank University processing the personal data (including sensitive personal data) you enter into this report in accordance with this privacy notice, the University’s Data Protection Policy and our Safeguarding Policy.

LSBU uses Report and Support to collect and store data.  Report and Support is a specialist third party system created specifically for universities to use for this purpose.  The system was designed to ensure the security of data – you can find out more about how Report and Support keeps your data safe here.

We may use the information you provide for the purpose of:

  • Helping us to identify the best person for you to speak to in relation to your report;
  • Providing the person with your contact details (if you have provided them) and some useful background information that will enable them to understand the nature of your concern or the incident you are reporting;
  • Monitoring patterns or trends in data that will be used to inform our proactive and preventative work. We will keep an anonymised record of your report to identify whether there have been/are other similar reports and whether there is a pattern of behaviour or trends that should be addressed.

Making a report does not initiate a formal process, and we will not take any action until we have spoken with you to discuss your report. We will not disclose any personal or identifiable information to any other party unless we believe there is a genuine threat to a person's health and safety.

At any time, you may ask that we stop processing your personal data and that we safely and securely delete the information you gave us.

Your personal data will be shared within London South Bank University on a need to know basis only so that people who need to know and act have the right information to respond to your report.  Other than being stored on Report and Support (see above) your data will not be disclosed to any third parties without your consent unless there are legitimate reasons requiring us to do so, for example, where the information you have provided highlights a potential risk to a person’s health and safety.

The University's Data Protection Policy and Privacy Notices provide further information about the lawful basis on which we operate,  as well as your rights and how we protect your privacy. You have the right to withdraw your consent and request the deletion of your personal data at any point by contacting the Data Protection Officer at dpa@lsbu.ac.uk.
